Please ensure to add information for children and parent/caregiver. 1) Fill out the required fields, but DO NOT HIT COMPLETE 2) Click "Add Family Member." Fill out the required fields for an adult. DO NOT HIT COMPLETE. 3) If you have another child, click "Add Family Member" and fill out required fields for a child. DO NOT HIT COMPLETE- Click "Add to List" in bottom right corner. 4) After ALL family members have been added, click "Complete Signup"
