Waitlist Instructions

This form is HIPAA compliant. Please complete the online Case History Form to its entirety. Once the online Case History Form is received, you will be automatically added to our waitlist. If you have reports from another agency, please email a copy to clinicassist@mail.fresnostate.edu. Thank you for your interest in Fresno State-Scottish Rite Speech and Language Clinic and the CSDS Research Clinic! The Fresno State-Scottish Rite Speech and Language Clinic and the CSDS Research Clinics are dedicated to serving people with a variety of communication issues in the community. Evaluations and treatment are available for people of all ages. Please note: *Our clinic operates on a first come first served basis and has a waiting period of 6-9 months. *Please note that no clinic spot is guaranteed. All placements are subject to the clinic director's approval and are based on client and student clinician needs and available spots. *The office staff will call you when a spot is available. *Clients can self-refer. *All clinic services are free of charge.
