Waitlist Instructions

Welcome to the evaluation waitlist for Freedom First Psychological Services, PLLC. This form is for requests related to comprehensive Psychological Evaluation and testing for diagnostic purposes. Please fill out the form below to express your interest in a psychological evaluation.

There is currently a 2-3 month waitlist for both child and adult evaluations. We are only able to schedule evaluation intake appointments up to one month in advance. Once you have reached the top of the waitlist, you will be receiving appointment options for that month or the following month.

Important to note:

We do not complete court mandated evaluations, military or employment screenings, or provide medication management.

This is not a crisis evaluation service. If you are experiencing severe mental health symptoms and are in need of immediate support, please call 911, call 988, or go to your local emergency room.

A Psychological Evaluation is an intensive, 2-4 appointment process for diagnostic purposes, different from ongoing Therapy or Mental Health Counseling Services. If you are only looking to begin Therapy Services, please do not fill out this form and instead, please visit the Therapy Services page of our website, https://www.freedomfirstpsych.com/therapy-services to fill out the New Therapy Client Form. Psychological evaluations are not required to begin therapy. If you are interested in both evaluation and therapy services, please fill out both this form and the New Therapy Client form to be added to each waitlist.

We do not provide first time autism testing for anyone over age 15. Anyone over the age of 15 seeking autism testing will need to have an existing autism diagnosis and be seeking an updated evaluation for enrollment in services (such as OPWDD). We will accept one of the following as proof of diagnosis: 1) a copy of a previous evaluation in which the autism diagnosis is listed or 2) a signed letter from a psychiatrist or primary care physician confirming the current autism diagnosis. We will not be able to schedule the intake appointment without this documentation.

After completing this form, you will receive an automatic confirmation email from alert@waitlistplus.com that this form has been received. This email will include a link to access your client portal, where you can check your status or remove yourself from the waitlist at any time. We recommend saving this email address as a contact to prevent scheduling emails from being sent to spam.

Our staff will reach out with any further questions or for clarification if needed after reviewing your submission. Otherwise, you will receive an email when it is time to schedule an appointment.
